This game already has a ton of end game, just because you have done it, or choose not to do it IE Item grail doesnt mean it doesnt have it. You already done everything in the endgame? And i mean everything. Did you do the game from bottom only playing with what you loot? There is endgame, but if you follow guides on internet, follow build, powerxp with group and take free item from other, yes, you will be bored at a time.
Once you get a character maxed out you switch to working on your next character. That is what d2 always was. I still have a few characters that I want to run through the game. Another option that some players choose is pvp, although I have never personally been a fan of the PvP in d2. Lastly there are sites that host tournaments with specific rules, ie how far can you make it with only using wirts leg in hc and not being able to clear the same area more than once.
